Personalised supports within the context of a community supports model describe ways of supporting people with dementia to maximise their abilities and to remain living full and active lives, giving equal weight to the range of clinical/medical services and societal/community supports that are required.
A personalised response recognises that a wider ecosystem of care and support inclusive of the person, their family, a wider social network, mainstream organisations, and health and social care can better support the person with dementia to live a full life in their community.
